The primary goal of this practice is to deliver quality care in an efficient and personalized manner in collaboration with the patient and family or other caregivers.

  • Decreases acute episodes or requirement for emergency care

Collaborative Care:

  • ability of patients to provide feedback to improve practice

  • ability of patient to move seamlessly among providers engaged in different aspects of their care

As a practice we will measure our processes, outcomes, and patient experiences in an attempt to meet these goals. Together we will develop plans and goals at each visit, so we can measure our success in ways that are meaningful and actionable.

In addition to direct patient care goals, we aim to:

  • Contribute to the training and education of premedical and medical students and residents

  • Be environmentally friendly

  • Engage with the community

  • Support legislation that will positively impact children and adolescents and healthcare delivery
