First Visit

THIS IS A DIRECT CARE SPECIALTY PRACTICE. INSURANCE IS NO LONGER ACCEPTED FOR VISITS. FAQ

PRACTICE REGISTRATION FEE $50

Charged at time of booking the first appointment. Non-refundable.

A credit card is necessary to enroll in the practice and will be kept on file and used for all charges.

NEW PATIENT VISIT FEE

≤ 90 minute visit $330

90-120 minute visit $350

≥ 120 minute visit $410

Includes one 15 minute follow up call/televisit within 4 weeks of initial visit. Billed at the conclusion of the first visit.

Televisits/follow up calls/emails not completed within 4 weeks of the first visit are not included in the new patient visit fee and will be billed separately as outlined in the Fee for Service Follow Up Schedule.


Follow Up: “Monthly Fees” or “Fee for Service”

WHICH ONE IS RIGHT FOR ME? 

If Dr. Bardsley prescribes any medications, choosing the Monthly Fees will be more convenient and cost effective than being billed for each visit, communication, and prescription refill. If you are not sure if follow up is needed or if there are no prescriptions, then the Fee for Service may be better.


Monthly Fees

Monthly fees are labeled as “Membership Fees” in the Patient Agreement.  They begin 6 weeks after a new patient visit if monthly fees are chosen by the patient. The appropriate tier will be discussed with Dr. Bardsley.   

 
  • 1 Visit every 9-12 months

    Prescription refills as appropriate, excluding subcutaneous injections, controlled substances,  and diabetes supplies

    Bloodwork and imaging interpretation as necessary

    Unlimited texting/direct physician access

    Patients who might be well suited to this tier are those who have been followed by Dr. Bardsley for years and are stable, need only 1 visit per year but still require prescriptions and would like to have the ability to email/text or call without being charged for an occasional check-in. 

  • Includes services for Tier 1 plus

    1 Visit every 6 months (2 visits per year)

    Up to 30 min per recommended follow up interval spent completing insurance/prescription prior authorizations/faxing

    Prescription refills as appropriate, excluding subcutaneous injections, diabetes supplies, and controlled substances

  • Includes services for Tier 2 plus

    1 Visit every 4 months

    Refills include controlled substances but exclude growth hormone and other subQ injections

  • Includes services for Tier 3 plus

    Unlimited Visits

    In office hemoglobin A1c 4 times a year 

    Unlimited remote blood sugar interpretation

    Intramuscular injections   

    Prescription refils as appropriate

 

Fee for Service

Fees for services for patients who do not choose the monthly fee schedule and for services above and beyond what is included in the monthly fee schedule “membership” tier. 

 
  • 0-30 minute visit $280

    30-60 minute visit $330

    60-90 minute visit $400

    Injection + vitals $150

    Hemoglobin A1c $25

    Hourly Fee** $300/hr

  • Prescription refills require follow-up visits and in some cases bloodwork or imaging as recommended.

    Refills will be written for at the visit to cover the period between recommended visits. 

  • Hemoglobin A1c in office: $25/test

    Injection: $100

  • $300/hr

    Telephone calls/texts/emails/blood sugar interpretation/insurance/Prior Authorizations/ or other tasks outside of face to face time exceeding 20 minutes time per recommended visit interval

    Hourly fees are calculated using 1/10 of an hour (6 min), ⅙ of an hour (10 min), ¼ of an hour (15 min) 

 

Submitting for Insurance Reimbursement 

When asked, I will provide a claim form for submission to insurance. Your plan’s OUT OF NETWORK coverage and deductibles will determine whether or not you are reimbursed, and how much.
